Cases documented in forensic literature are typically associated with severe psychopathology, including psychotic disorders, extreme personality pathology, and profound empathy deficits. Research on sexual fantasy consistently shows that people may have fantasies about scenarios they would never act upon and would find horrifying in reality. It should be noted that vorarephilia (the fantasy of swallowing or being swallowed whole) represents a related but distinct and less extreme fantasy interest that does not involve actual violence.

Christopher Columbus recorded the word based on accounts of the Carib (Karib) people, whom neighboring groups accused of consuming human flesh. The term became generalized to describe the practice of humans eating human flesh.

Cannibalism has occurred throughout human history in various contexts: survival situations, warfare, ritual and religious practices, and criminal behavior. The sexual dimension of cannibalism has been documented in rare criminal cases throughout history, with notable cases attracting significant public attention and contributing to both forensic understanding and cultural mythology.

The study of cannibalistic fantasies and behavior within sexology and forensic psychiatry developed primarily in the 19th and 20th centuries. Krafft-Ebing's Psychopathia Sexualis included references to cases involving the intersection of sexual desire and cannibalistic impulses. Contemporary forensic psychology continues to study these extremely rare cases to improve risk assessment, profiling, and intervention strategies.
