Within some sexual subcultures, barebacking has been embraced as a deliberate preference, with practitioners citing the increased physical sensation, intimacy, and psychological connection of unprotected sex.

Without barrier protection, the risk of transmitting STIs including HIV, gonorrhea, chlamydia, syphilis, and hepatitis is substantially increased. Discussions about bareback sex require nuance and should account for the full range of risk reduction options available, including regular STI testing, PrEP, knowledge of partner health status, and the use of treatment as prevention. The term bareback originates from equestrian vocabulary, where it describes riding a horse without a saddle. Its application to unprotected sex emerged in the gay male community during the 1990s, initially as slang to describe intentional unprotected anal sex.

The term gained prominence during the height of the AIDS epidemic, when the choice to forgo condoms became a charged topic within gay communities. The bareback subculture that emerged was controversial, with public health advocates raising concerns about HIV transmission while some community members asserted their right to make informed choices about their own sexual practices.

The ongoing evolution of HIV prevention tools, including PrEP and U=U, has continued to reshape the conversation around bareback sex.
